Lookup previously entered dog using web entry code or dog info

INSTRUCTIONS:   After submitting an entry, a web reference code "Picalo Code" is provided that can be used to access a dog record for additional entries. This code is also provided with the confirmation email with each online entry.

If the code locates the dog record, the "USE THIS DOG" button will bring up a new blank Entry Form Screen with the dog information pre-filled.

Previous information on specific entries is not included, only the dog information that can then be used for a new entry.

If the code does not locate a dog record, the "GO TO ENTRY FORM" button will bring up a new blank Entry Form Screen.

	Due to Privacy Considerations, Only an Entry Code will Include the Address, Phone Number and Email Information

REG NUMBER:	Search Order: Any or all fields can be used. Search is from top field (code) to bottom. Partial matches are acceptable for Dog Name, but only one found record is displayed.
